Submit Your Form - Two Options

The "Volunteers" option on the top menu includes the following forms that can be completed and submitted online:

• Volunteer Questionnaire: Complete this form if you're interested in volunteering, and someone will contact you to discuss opportunities.
• Team Application: Complete this form if you are ready to apply to join a specific team for an event listed here.
• Day Angel Application
• Spiritual Director Application

If you're unable to submit your form online, you may

1. Download and print your form from the volunteer forms listed below. 
2. Mail your completed form to the Kairos Outside North Carolina location closest to you. If uncertain which address to use, mail your form to Northeast address, and we will make sure your form gets to the correct location.
